Please download a lab card or call us and we can send you some, fill in lab card and try to be as descriptive as you can in your instructions
Step Two
Pack impression and lab card into package, please feel free include photos, diagrams and anything else you think may help.
Step Three
If local please call and we will collect, otherwise we recommend using Australia post, or a courier service such as Couriers Please to send the case to us.
